Suro Health Pty Ltd (ACN 110 446 234) (Suro Health) is committed to treating your personal information in accordance with privacy law. This Privacy Policy sets out Suro Health’s privacy obligations to you and explains the types of personal information that we may collect, hold, use and with whom the information is shared. What personal information we collect and store: Depending on the individual circumstances, the types of personal information that we may collect include contact information (such as your name, date of birth, address, email address, telephone number, fax number, emergency contact information), company name and position title, health information and particulars of health services provided.
How we collect personal information: Personal information may be collected by Suro Health from our clients and examinees, as part of the service we provide, in person, via the phone, forms and during telehealth appointments. Where possible, reasonable and practical to do so, we will collect your personal information directly from you, in person, in writing, over the phone, through text messages, by facsimile or by email. We may be recording and storing: (a) online consultations (audio and/or visual) via various computer technology and/or (b) online/in-person consultations via CCTV cameras (visual only), both for quality assurance, dispute resolution and similar purposes.

The purposes for which we collect, hold and use personal information: All personal information collected by Suro Health is used solely for our business functions and activities. It may be used for the purposes of allowing Suro Health:
We will only request information that we believe to be reasonably necessary for us to perform our key functions and activities, and that is necessary to allow our specialists to provide a comprehensive assessment and opinion. The personal information that we collect will be disclosed in the medical reports we provide to our referring clients (third parties who referred you to us).
Disclosure of personal information: In the course of undertaking Suro Health’s key functions and activities, it may be necessary for us to use or disclose your personal information to our employees and other parties including the organisation or individuals who referred you to our services, to the medical providers and specialists that we engage to provide services, as well as to other third parties and subcontractors that provide services to us. Suro Health will only use or disclose your personal information for the primary purpose for which it was collected, or a directly related secondary purpose you would reasonably expect. Where we need to use or disclose your personal information for a secondary purpose, unless otherwise required or permitted by law, we will only do so with your consent.
Security of personal information Suro Health follows privacy control measures in order to protect your personal information from misuse, interference, and loss, or from unauthorised access, modification ordisclosure. Suro Health may hold information in both hardcopy and electronic forms.
How you may access and correct your personal information: Suro Health’s suite of products and services are provided to our referring clients. If you were referred to our service by another organisation, we recommend that you approach that organisation directly with requests for access to, or revision of, your personal information.
Under the APPs, you may also be able to obtain a copy of the personal information that we hold about you. The APPs provide some exceptions to your rights in this regard. To make a request to access this information, please contact us in writing to one of the contact addresses below. We will require you to verify your identity and to provide as much detail as possible regarding the information you require and the form in which you wish the information to be provided. We may charge a reasonable fee to cover the cost of verifying the application and locating, retrieving, reviewing and copying any material requested.
If Suro Health deems that your personal information is of a sensitive nature (where we have concerns about your health, wellbeing or safety or other special circumstances exist), then your relevant personal information may be released to your General Practitioner or treating health care provider only.
We endeavour to ensure that the personal information we hold about you is accurate, complete and upto-date. Please contact Suro Health if you believe that the information we hold about you requires correction or is out-of-date. If we are unable to correct your personal information as requested, we will provide you a written notice with the reasons for the refusal and the mechanisms available to complain about the refusal if you are not satisfied with our decision. Further, if we are unable to make the correction, and at your request, we will take such steps as are reasonable in the circumstances to associate with the information, a statement that you believe the information is inaccurate, out-of-date, irrelevant or misleading and details of the amendment sought.
We will endeavour to respond to written requests within 30 business days after a request is received by us.

Suro Health will investigate the complaint and will endeavour to respond to you within 30 business days. Suro Health will take immediate steps to redress proven privacy concerns or breaches.
